At Functional Physical Therapy & Fitness, we make sure our patients play an active role in their recovery so they can regain their function again once treatment ends. We understand that no two people or injuries are the same, which is why we create an individualized treatment plan aimed at achieving the functional goals we set together.
Your physical therapy treatment typically begins with an initial one-on-one evaluation session with our licensed physical therapist, who performs a thorough assessment to identify the root cause of your condition and prescribe the best course of your treatment. We also examine your posture, movement, and daily activities to advise you with corrective postural and ergonomic re-education to improve your condition, reduce your pain, and prevent future injuries. Your session may also include manual therapy, joint mobilization and manipulation, postural re-education, as well as therapeutic corrective exercises to strengthen your muscles and increase your range of motion.
Our physical therapists are highly experienced in treating many types of injuries. We offer a wide range of rehabilitative care treatments for orthopedic injury, post-operative surgery, sports injury, workplace injury, motor vehicle accident, and neurological injury. We also work closely with your physician or surgeon to monitor and update your rehabilitation progress during the course of your treatment.
We accept PPO, Optum HMO, Medicare, and Workers Compensation insurance plans. We also accept patients who prefer to pay out-of-pocket at our special cash rate. We accept both cash and debit/credit card payments. Please contact us directly to inquire about our rate or to verify whether or not we are part of your insurance network.
We welcome patients with physician referrals. To expedite your visit, new patients are encouraged to fill out our new patient form package prior to coming to our office. Please download the New Patient Forms in the link below and bring them to your first appointment along with your doctor's prescription, insurance information, and photo identification.
